hit wall on first exercise
I'm beginning to wonder if I just purchased an obsolete edition of the book. There's no reference to edition #2, #3, etc. The copyright is 2002.

Anyway, on P4, #2, how do you adjust the position (or shape) of the inner ellipse - either with the mouse still clicked or after you unclick the mouse? Since ALT is pressed, you can't use the spacebar tip and the arrow keys seem to move both ellipses.

p4, #7/8 - if you hold alt to create a 'subtracted region', you don't get anything like the diagram above #7.

I want to make sure I'm not missing something fundamental...

Hmm, looks like there's a bug in the s/w. I submitted this question and it came back that I had to have a valid email first. I then updated my email and went back and then saved the post and ended up with two.

On using the pen, p10, whenever I go from point to point, PS creates a filled (forground color) triangle which overwrites the photo of the cup.

It sounds like you have the first edition, which is not obsolete, just original!

Did you hold ALT prior to making the selection, if not, that could be your problem. Modifiers behave in different ways, depending on when you apply them.

For the second question, is quick mask set to show masked or selected areas (see the intro of page 6)?

You need to set it to path mode as you currently have it in filled shape mode - it's at the top and shows the pen tool inside a four point square.
